Denne artikkelen har vi ventet lenge på, relevant som den er for nærmest alle spesialiteter.
I mange tilfeller har man tid til å vente en måned eller to før man starter immunsuppressiv behandling. For mange vil det være aktuelt å reise til eksotiske steder i fremtiden, og man har her siste sjanse til å ta gulfebervaksinen. Gulfebervaksinen er levende, og kontraindusert ved alle former for immunsuppressiv behandling. Vaksinen er obligatorisk ved innreise i mange land, oftest når man reiser via et land hvor det forekommer gulfeber. Det går an å utstede en fritaksattest, men vi vet ikke om innreiselandet vil akseptere den. Det er gode grunner til at gulfebervaksinen er obligatorisk, og fritaksattest er en nødløsning.
Det kan også være aktuelt å gi andre reisevaksiner før man blir immunsupprimert, for eksempel hepatitt A- og rabiesvaksine, i og med at immunsuppresjon kan svekke effekten av vaksiner. Levende tyfoidvaksine kan være aktuelt, særlig til personer som med stor sannsynlighet kommer til å reise til Pakistan.
Folkehelseinstituttet har holdt igjen når det gjelder anbefaling av vaksine mot denguefeber, og anbefaler den bare til folk som har hatt denguefeber før (1). Dette er en levende vaksine, som må gis som to doser med minst tre måneders mellomrom. Det innebærer at oppstart med immunsuppresjon måtte utsettes i fire måneder. En ny denguevaksine, som gis som én dose er registrert i Brasil (2). Risikoen for denguefeber per måned i endemiske områder er ca 1 av 500 (3), og dette er en sykdom som er på fremmars i store deler av verden. Folk som planlegger lange opphold i for eksempel Thailand burde overveie denguevaksine før eventuell oppstart av immunsuppressiv behandling.
